The science behind the work

Why concentrated, intensive work is so effective, the developmental foundations we are actually supporting underneath, and the role families play in creating lasting change.

Step One

Observe and consider

We start with the behaviour exactly as it shows up. When does it happen, and when does it not? What is being asked in that moment? How far into the day are we?

From there we consider the demands of the task, what your child may be thinking about that task or about their own capabilities, and how their nervous system responds.

“How do I organise my body?” “Does my work look as good?” “Does my play look organised?”

Aspects of Cognitive Behavioural Therapy are useful here. The task itself is one part of the challenge. Another is how a child feels they measure up against it, a perceived threat to their sense of achievement or self-esteem in front of others. That thought carries a physical response, and the behaviour that follows is the body protecting itself from the discomfort, often by leaving or by avoiding the task before it begins.

We also consider the nervous system driving that behaviour. It may be reading the situation as a threat and responding protectively. A child who cannot organise their body for a task may leave the table before anyone notices, or turn the moment into a joke. When there is no capacity left for those quieter strategies, the result is dysregulation, or a meltdown.

In these moments cognition becomes harder to access, and for our children this area is still developing. So we start underneath it, with the body and the nervous system.

Step Two

Integrate and regulate

These are the two areas we can work on most effectively at this age. Integrating the body changes what a moment costs. Regulating the nervous system gives a child access to everything sitting above it.

Where a task reads as threatening or challenging, whether writing at a desk, getting dressed or sitting through a meal, an occupational therapy assessment looks underneath the behaviour at the foundations supporting participation:

These are the skills that allow a child to feel organised in their body. Where some are still developing, an ordinary task costs far more effort than it appears to, which is what the nervous system reads as threatening.

As the body becomes more organised, the same task starts asking less of it. The behaviour underneath usually changes on its own, without anyone having to correct it.

RegulateThe nervous system

Alongside that, we scaffold regulation directly. Rather than expecting your child to manage their own nervous system, we let them experience regulation alongside us first, using input the body reads as calming and organising.

Deep pressure

Firm, contained input through the joints and body that helps an activated system settle.

Rhythmic movement

Steady, predictable movement the nervous system can organise itself around.

Heavy work

Pushing, pulling and carrying that gives the body strong, grounding feedback.

Co-regulation comes before self-regulation. A child borrows our steadiness for a long while before they can generate their own.

That integration begins with the reflexes and sensory systems that shape how your child moves, feels and engages.

Understanding Reflexes

What primitive reflexes are, and why they matter

Reflexes and sensory systems shape a child’s sense of safety, awareness of space, gait, capacity to sit and engage, transitions, social connection, handwriting and academic participation.

Early in life, the body is guided by automatic movement patterns called primitive reflexes. These are intended to integrate as a child grows, becoming the foundation for posture, coordination, attention and regulation.

When a reflex stays active, the body is quietly working harder underneath the surface. What can look like behaviour is often the nervous system asking for support.

Reflexes and the sensory systems are deeply interconnected. A retained reflex keeps a sensory system working overtime, and an overloaded sensory system makes it harder for a reflex to settle, so we rarely address one without the other.

Moro The body's first protective response to threat and change. Startles easily, big or quick emotional reactions, sensitive to loud sounds and busy places, finds change and transitions hard.
ATNR Links head turning with the arms and eyes. Messy or effortful handwriting, struggles to copy from the board, loses place when reading, finds crossing the midline tricky.
STNR Separates the movement of the upper and lower body. Slumps or props at the desk, wraps legs around the chair, fidgets, tires quickly, finds sitting still really hard.
TLR Anchors posture and balance to gravity. Clumsy or trips often, poor balance, slouched posture, unsure of where the body is in space, tires when standing.
Spinal Galant Drives early movement and wriggling through the trunk. Constant wriggling, can't sit still, sensitive to waistbands or clothing tags, fidgets when seated, may affect toileting.

As the body becomes more organised, children often show steadier posture, smoother coordination, more consistent attention, easier handwriting and more regulated responses.

Understanding the Sensory Systems

How your child takes in and makes sense of the world

Working alongside those reflexes, the sensory systems gather information and help the brain decide how to respond. When they are well organised, a child can stay calm, focused and engaged. When they are working harder, everyday moments can feel like too much, or not quite enough.

Sight & sound Processing what is seen and heard. Covers ears at loud sounds, overwhelmed in busy places, distracted by visual clutter.
Touch Making sense of textures and contact. Bothered by tags or seams, avoids messy play, or seeks constant touch and squeezes.
Balance (vestibular) Sensing movement and where the body is. Constantly on the move and seeking spins and swings, or cautious and unsure on the move.
Body awareness (proprioception) Knowing where the body is without looking. Bumps into things, uses too much or too little force, leans and crashes for input.
Interoception Noticing signals from inside the body. Misses hunger, thirst or toileting cues, finds it hard to name or notice feelings.

When the sensory systems are well supported, children feel safer, more comfortable in their body, and more available to learn, connect and engage.

Purposeful, Functional Play

Not just play, purposeful movement embedded through play

What looks like play is purposeful, functional movement. Each activity is chosen to target a specific reflex, sensory system or developmental goal, then embedded within play so your child stays engaged. Felt safety and engagement are what make the nervous system most open to change.

Drawing on aspects of DIR Floortime, family systems and attachment theory, I meet each child at their developmental level and build from what motivates them.

Research into relationship based approaches shows that engagement and felt safety strengthen gains in regulation and learning. The work is targeted and relationally attuned at once.

The Signature Approach

Why I recommend intensives

Reflex, sensory and movement work all require repetition before it holds. Each time the body repeats a movement, the brain strengthens the pathway behind it. Pathways used often become efficient, while those used rarely stay effortful. Research in motor learning indicates the brain learns best through frequent, focused repetition.

That is the thinking behind an intensive model. When practice is concentrated into a shorter window, each session builds on the last while the learning is still fresh, and the foundations receive the repetition they need to consolidate. The foundations are laid first, because everything a child does afterwards is built on top of them.

The brain responds to repetition and frequency, not time alone.

Repetition
new motor patterns are built through repetition, using everyday activities that need no clinic and no equipment.
Purposeful movement
every activity has a clear clinical target, chosen deliberately and delivered so it feels like play rather than practice.
Intensity
frequent, closely spaced practice is what drives motor learning most effectively.

This is why the intensive is the signature approach. It is designed around how learning and the nervous system respond to focused, repeated practice. Every child is different, so we always tailor the pace to the individual.

Working Together

The importance of parent collaboration

The most lasting growth happens not only in sessions, but in everyday moments at home. Families are central to this work.

Within an intensive, collaboration extends the work between sessions. Where a family chooses individual sessions it matters more still, because the daily home activities carry the progress.

Completing those activities each day, embedded into ordinary routine, gives the nervous system the repetition it needs.

However we work together, a little, woven into each day, builds the foundations that last. You are supported throughout with a clear Individualised Implementation Plan and the coaching to use it confidently.
